Prior to your child(ren) entering the daycare, all members of the household must complete a COVID-19
self assessment using the COVID-19 screening form below.
If anyone has even one COVID-19 symptom, or been in close contact with someone who tested positive for
COVID-19, or travelled, everyone must stay home, self-isolate and arrange for testing.

If you’ve been away for anything related to Covid-19, we now require that you use the Return to Child Care Confirmation Form, below.
We have updated many of our policies and procedures to ensure compliance with the COVID directives set out by Toronto Public Health and Ministry of Education. We will also follow all regulations set out by TDSB.
